Add a changelog

Apologies if I've missed it somewhere.

It would be great if this project maintained a changelog somehow. This might be a CHANGELOG.md somewhere in the repo, perhaps published into the docs, or just notes on annotated tags or on releases on GitHub. Even just a section in the README with descriptions of the major versions.

This would help developers understand when it's safe to upgrade between versions.
